If you are in the process of listing, selling or refinancing your condominium unit, or if you are a mortgage broker in need of condominium details, please download and submit the below form. The form must be signed by the unit owner(s).

A 6(d) certificate is often required at the time of closing for the sale or refinance of a condominium unit. Our normal turnaround time for completing 6(d) requests is ten business days from the receipt of a completed order form.

If you are an appraiser and need condominium-related information, the owner of the unit must authorize Lundgren Management to release certain information to a third party. Please download the below form and have it signed by the unit owner(s) before submitting it to Lundgren Management.
