A 6(d) certificate is often required for the sale or refinance of a condominium unit. Our normal turnaround time for completing 6(d) requests is ten business days from the receipt of a completed order form and the associated fee.

To request a 6(d) certificate, please print the 6(d) Order Form and deliver the completed request along with payment to the Lundgren Management office in Chelsea.

This same form may also be used to order condominium documents and copies of recent
meeting minutes.
